Skilled nursing care offers continued treatment to those individuals who may need additional rehabilitation following a hospital stay. The goal of skilled nursing care is to restore the patient to their highest level of independence and functioning as quickly as possible. With skilled nursing care, you are under the observation of a physician who is responsible for an overall care plan and supervises your progress.

St. Joseph Health has a large skilled nursing care network throughout the region, with multiple locations. Our comprehensive team of physicians, nurses, social workers, and chaplains work together to create the best treatment plan unique to each individual.

Skilled nursing care is a choice for you, your family and your physician. To facilitate the process at any St. Joseph Health facility, work with your case manager and physician.

Reasons for Skilled Nursing

Individuals may need 24-hour nursing care following a surgical procedure or extended illness. Some things that skilled nursing care can assist with include:

  • Wound care and dressing changes
  • Administering and monitoring IV antibiotics
  • Nutritional therapy
  • Physical & occupational therapy
  • Respiratory therapy
  • Rehabilitation and recovery from surgery

Private insurance and Medicare may pay for skilled nursing care for certain time periods following a hospital stay. A representative from our case management team or patient financial services team can advise you on your eligibility.
